of Items to have for Mortgage The following checklist will help you
to collect all the documentation that
you will need for the loan application.
It is also a good idea to bring copies
of these documents to your loan application
appointment. If you are not able to
bring all the information to your appointment,
or are applying online, mail the documents
to the attention of your Loan Officer.
To Verify Income:
Paycheck stubs covering the most recent
30 days from each borrower
Federal tax returns or 1040s for the
most recent two years
W-2s or 1099s for the previous two years
Bank/retirement/investment statements
covering the most recent two months
for all accounts
Disability, Social Security, or retirement
pensions–copy of the awards letter
or other documentation of income received
from these sources, including evidence
of continuance
Fax number of Human Resources department
from your current employer
To Verify Housing Information:
Rental information needs to be provided
if you have rented housing in the last
two years. The following information
should be provided: property address,
dates of residence, landlord’s
name, address and phone number
The Disposition of Real Estate &
Estimated Net Proceeds form from the
sale of your current residence, if applicable.
Please ask your assigned Loan Officer
or Loan Processor if this form is needed
To Verify Account Information:
Retirement account statements for the
previous two months
Bank account statements for the previous
two months on all accounts listed on
the application
Gift letter will need to accompany the
application if gift funds will be used
for the down payment or closing costs.

To Verify Income If You Are Self-Employed:
K-1 Schedule of your tax returns for
the previous two years if you have an
interest in a limited partnership
Partnership tax returns (IRS form 1065)
of your tax returns for the previous
two years if you have a 25% or greater
interest in a partnership
Signed year-to-date profit and loss
statements for business
Schedule C of your tax returns for the
previous two years if you are involved
with a sole proprietorship
Corporate tax returns (IRS form 1130
or 1120s) of your tax returns for the
previous two years if you have 25% or
greater interest in a corporation or
S corporation
Miscellaneous Information To Verify:
Child support or alimony–copies
of 12 months of documentation showing
income received and evidence of continuance
Divorce decree–copy of complete,
final decree and any other applicable
documents
Bankruptcy/discharge of debtors–copies
of documents, including all schedules
Relocation papers showing start date,
pay and company buyout, if any
